Information About The Person Providing Visa Letter in Chile

• Complete name of the person in Chile
• Date of birth of the person in Chile
• Home address and contact number in Chile
• Occupation of the individual in Chile
• Whether you’re national of Chile or Long-term Resident
• A photocopy of a document confirming person’s residential status in Chile. (eg: copy of a birth certificate if the individual born in Chile or has Chilean citizenship card.)
