Information About The Person Providing Visa Letter in USA

• Complete full name of the person in USA
• Date of birth of the person in USA
• Residential address and phone number in USA
• Profession of the individual in USA
• Whether you are national of USA or Permanent Resident
• A photocopy of a document proving individual’s residential status in USA. (eg: copy of a birth certificate if the individual born in USA or has American citizenship card.)
